Forum on Religion, Spirituality & Aging Award

Purpose of the Award
The Forum on Religion, Spirituality and Aging (FoRSA) of the American Society on Aging (ASA), is pleased to announce its Religion, Spirituality and Aging Award, recognizing outstanding individuals, programs, and services in religion, spirituality and aging.
In 2010, the Religion, Spirituality and Aging Award will be given to an outstanding individual or organization who has demonstrated continued excellence and leadership in direct or indirect service to the religious and spiritual needs of older adults and their families and who has had a profound and lasting impact on the religious and spiritual concerns of older adults, through practice, teaching, research, advocacy or policy planning.
The winner will be honored by FoRSA as part of the
2010 Aging in America Conference.

Eligibility
Candidates must be members of the American Society on Aging before the deadline for submission. Candidates not currently a member may apply now at www.asaging.org/join.
Nominations may be made by ASA members and staff or by other professionals in the fields of religion, spirituality and aging.

Selection Criteria
Nominees will be judged on the basis of:
- How long has the nominee worked in the field of religion, spirituality and aging? 1-5 years? 5-10 years? 10-15 years? Over 15 years?
- What significant contributions or exemplary service to the religious and spiritual needs of older adults has the nominee demonstrated in the area of health services, research, education, policy planning, advocacy or congregational activity? Give specific examples of such contributions/service.
- How has the nominee demonstrated leadership and innovation in the field of religion, spirituality and aging? Give specific examples.
- How has the nominee demonstrated effectiveness in meeting the religious and spiritual care needs of older adults? What has the nominee added to the knowledge, skills/behaviors in one or more of the following areas: research, teaching, congregational work, clergy training or chaplaincy services?
- What impact has the nominee’s service/contributions had locally, state-wide, nationally, internationally? Give specific examples.
- If applicable, how has the nominee’s service or contributions benefited diverse or underserved populations?

Materials to Be Submitted
- Nomination letter summarizing the work and accomplishments of the nominee in the religion, spirituality and aging field and outlining how the nominee meets the award criteria outlined above..
- A statement from the nominee indicating willingness to be nominated.
- Résumé or CV highlighting nominee's accomplishments in the field of religion, spirituality and aging.
- Two letters of support indicating how the nominee meets award criteria
