Executive Summary
This survey was designed to:
Establish a benchmark of current Health Promotion/Disease Prevention (HPDP) programs and services provided by a sampling of professionals in health, aging and older adult housing networks.
Assess the need for HPDP information, tools, and programs for specific conditions and populations, and identify barriers to health promotion programs.
Guide the development and distribution of HPDP materials and tools responsive to specific information gaps, strategies and resource needs as identified by the surveyed organizations.
Acquire a profile of the providers and their clients who could benefit from this program.
The survey was conducted during the summer and fall of 2000.
