Please complete this online form within 60 days of the live date of each web seminar. You can attend the live event or take advantage of the recorded program to claim your free CEU credit (thanks to Home Instead Senior Care). There is a maximum of one Board selection per person per event. CEUs must be claimed within 60 days of the live event, and the recordings are available for 60 days after each live web seminar. Your CEU credit will be mailed within 30 days of survey submission.

You can attend the live event or take advantage of the recorded program to claim your free CEU credit (thanks to the MetLife Foundation). There is a maximum of one Board selection per person per event. CEUs must be claimed within 30 days of the live event, and the recordings are available for 30 days after each live web seminar. Your CEU credit will be emailed within 30 days of survey submission.
