Cook County is fully engaged in a Court supervised effort to move 1,100 people from institutional settings to community living.  This effort may provide a preview of how service coordination will work in the future. Part of the federal Money Follows the Person, “Pathways to Community Living” in Chicago and suburban Cook County, it is an orchestrated effort to educate, encourage, and support people who would otherwise not be able to change their setting.  The Program reaches out and approaches every hurdle by pulling together the assistance of a variety of community based organizations and State Departments. 

Learn about the Colbert Decree as administered by the Illinois Department on Aging, Transition Engagement Specialists, Peer Educators,  Managed Care Organization health and long term services and supports, Housing Assistance and Community Mental Health relocation services in a program that reflects all the actors now involved in making independent living a reality.
