This web conference is the second in a series related to payer-provider partnerships. This session focuses on the successful integration of the Advanced Illness Management Program in the North Shore-LIJ Health System through several different financing models. Participants will learn how North Shore LIJ worked directly with payers and worked through its risk-based contracts to ensure that team-based palliative care in the community was supported and available to those in need. Dr. Kristofer Smith, VP and Medical Director, Care Solutions North Shore-LIJ Health System, will discuss the challenges faced and opportunities discovered as he worked to collaborate with several payers to support Advanced Illness Management services to people with complex care needs.
