The Gary and Mary West Senior Dental Center is a nonprofit, integrated community dental center dedicated to serving low-income older adults in downtown San Diego. The Senior Dental Center links high-quality and affordable oral healthcare with a suite of nutrition, case management and wellness services offered through Serving Seniors’ Gary and Mary West Senior Wellness Center, creating a one-of-a-kind, integrated community-based system of care for older adults. This web seminar will provide an update on this unique model of care and discuss how to best leverage community resource partnerships to improve holistic care for elders.

  • Understand how a holistic and comprehensive model of care combining access to high-quality, affordable dental care with other health and social services can deliver better oral healthcare for older adults;
  • Describe how seamless care coordination between more than 30 nonprofits at Serving Senior’s Gary and Mary West Senior Wellness Center can improve overall health for low-income older adults;
  • Explore how community resources can be leveraged to better address oral and overall health and wellness needs for low-income older adults; and,
  • Explain the collaboration between the Dental Lifeline Network’s DDS program and the Gary and Mary West Senior Dental Center as a means for expanding the availability of dental services for low-income older adults.
