The costs of hospital readmissions—physical, financial, and emotional—are staggering. If you have a client who is about to return home from a hospital stay, or are a caregiver to an older adult in this situation, this web seminar will help to avoid unnecessary readmissions through preparing the home (and yourself) for the older adult’s return. Crucial aspects on what to expect the first day—and the first month—after they return home will also be detailed. 

 Participants in this web seminar will be able to:

  • Describe four ways to prepare early for an older adult’s return home from a hospital stay;
  • Identify five important tasks to be completed in the first hours after an older adult returns from the hospital; and,
  • Pinpoint the four best transitional care points for an older adult’s first 30 days back at home.

Presenter:

Lakelyn Hogan is the Caregiver Advocate for Home Instead Senior Care. She holds a Graduate Certificate in Gerontology and is currently working toward a Master of Arts in Social Gerontology. She also holds a Master’s in Business Administration and her undergraduate education focused on Marketing and Communications Studies. Lakelyn has professional experience in the private and public sectors of senior care services. She has worked on special projects for the University of Nebraska–Omaha’s Department of Gerontology and the local Area Agency on Aging. She is also active in her local chapter of the Alzheimer’s Association. Lakelyn has experience assisting senior clients and helping their families navigate the senior care continuum. She is passionate about helping others, especially aging adults and their families.
