It can be nerve-wracking when an older adult is admitted to a hospital or healthcare facility. And it’s also likely that family caregivers aren’t thinking about what will happen when their loved one is discharged. However, planning for the returning-home process, right from the start, is one of the best things families can do to help set their loved one up for success. Many issues factor into why older adults may be vulnerable to problems at home after they have been in a medical or rehabilitation setting. This month’s web seminar will provide professionals with information to help families plan ahead and prepare for a safe return home.

Participants in this web seminar will be able to:

  • Discuss the importance of planning ahead for discharge;
  • List the risk factors that could cause a readmission;
  • Identify five key steps to help with a successful transition home; and,
  • Describe resources to share with older adults and their families.

Presenter:

Lakelyn Hogan is a gerontologist and caregiver advocate for Home Instead Senior Care and a doctoral candidate at the University of Nebraska Omaha, studying social gerontology. She has a master of arts in social gerontology and master’s in business administration from UNO. Lakelyn has professional experience in the private and public sectors of senior care services.
